Reversing and remanding the district court's affirmation of the denial of Social Security Disability and Supplemental Security Income benefits to a man with several mental illnesses that included bipolar disorder, antisocial personality disorder, and ADHD because the administrative law judge incorrectly characterized some evidence as 'self-report,' wrongly weighted the opinions of examining and non-examining physicians, and otherwise improperly assessed the claim.
