Treating physician's opinion that claimant was disabled was based on an extensive treatment history and entirely consistent with the findings of past treating physicians; allegations of limitation were evidenced by subjective complaints of physical and mental distress; fact that claimant could engage in some life activities does not support a finding that she retains the ability to work; remanded.
