The trial court's order denying wife's request to require husband to pay for her attorney fees and costs is reversed and remanded, where: 1) the trial court erred in denying the request because wife did not file Judicial Council form FL-319 which the court believed was mandatory; and 2) that form is not mandatory when, as here, comparable declarations are submitted.
