Juvenile court's dispositional order granting custody of minor to his biological father is reversed and remanded, where: 1) the appeal is not rendered moot by the court's later order removing minor from biological father's placement; 2) because biological father was a mere biological father, he was not entitled to presumed father status, and thus, the order granting him custody was in error; and 3) the visitation order failed to adequately specify the frequency and duration of mother's visits.
