ORDER

Pursuant to the provisions of S.C. Const. Art. V § 4, the Juror Summons for Circuit Court Form SCCA/235, bearing a revision date of 11/2016, is approved for use in the Circuit Courts of the State of South Carolina, effective November 1, 2016.

The juror's phone number blanks on the form have been revised. The section indicating “Home Phone #” has been renamed “Phone #,” and the section indicating “Spouse's Business Phone #” has been renamed “Alternate Phone #.” Form SCCA/235 (11/2016) supersedes and replaces Form SCCA/235 bearing a revision date of 12/2012.

This form will be available on the South Carolina Judicial Department website at www.sccourts.org/forms.
